Our Patient and Family Advisory Committee

Are you interested in getting involved in Mosaic PCN's Patient and Family Advisory Committee (PFAC)?

An essential element to high-quality health care is actively including the voice of patients and families.

  • Would you like to be a part of improving the quality and safety of health services?
  • Do you want to make a positive impact on health services in your community?
  • Have you had an experience with health care that you would like to see improved?
  • Would you like to share your opinion on what matters most to patients?
  • Can your health journey help shape positive experiences for future patients?

If you answered yes to any of the above then we'd like to hear your voice!

Volunteer today to be a Patient Advisor with Mosaic Primary Care Network.

What is a patient advisor?

A patient advisor is someone who wants to help improve the quality of care for all patients of Mosaic Primary Care Network. As an advisor you will have the opportunity to:

  • Provide feedback on our programs and services;
  • Help ensure our healthcare services respect the values and preferences of patients;
  • Share ideas on how to improve the patient experience;
  • Help people in our community better navigate the healthcare system

Who can be a patient advisor?

You can be a patient advisor if you are a Mosaic Primary Care Network patient and willing to volunteer for two hours per month to meet with the committee and share your ideas. Family members or caregivers of patients are welcome too.
