Welcome to Mosaic Primary Care Network (PCN) social work services. You have been referred to our social work services by your Primary Care Provider (PCP) or another Mosaic team member. Our team works together to provide primary care support as part of your medical home. This document will help you learn what Mosaic PCN social workers do and how they can work with you to meet your health goals.
Appointments and What to Expect:
- Your first appointment with your social worker should already be scheduled. Please check your email or
contact us to ensure it’s booked. - We do not provide urgent or “walk-in” supports, an appointment is required for our services.
- To book an appointment, call 403-276-1555, and push one (1) to speak to the medical office assistant.
Cancellation and No-Show Policy:
If you cannot make a scheduled appointment, please call 403-276-1555 (extension 1) at least 24 hours in advance to cancel or reschedule. This allows us to offer that time to another patient who needs support. If you miss an appointment without cancelling or rescheduling, we will consider this a “no show”. After two (2) no-shows, you will be discharged from social work services and require a new referral from your PCP to be seen again.
Limits to Confidentiality and Sharing Information:
Your health privacy is the highest importance to us. We use an electronic medical record system (EMR) to chart your health appointments in accordance with the Health Information Act of Alberta. Information collected during your appointment may include the concerns you present, your health goals, provider recommendations and the outcomes. Your PCP receives a copy of all notes, assessment, or report that we write in your EMR. Beyond sharing your session notes with your PCP, what you discuss with us remains confidential.
There are three types of disclosures that we must report by law if shared with us:
- You are at risk of harming yourself or someone else.
- You report the abuse or neglect of a child/minor or vulnerable person (including adults with disability).
- Mosaic PCN’s records are subpoenaed by the courts.
Note:
- We require your signed permission to communicate and share information with other organizations on your behalf. Your provider will let you know if a Release of Information (ROI) is needed to support you.
- Social work does not provide direct financial resources. We help you connect with existing resources in the community
Role of your Mosaic PCN Social Worker:
- Assess your situation to uncover your strengths, needs, and the goal(s) you want to achieve.
- Provide information on available resources to address concerns like food insecurity, access to medication, and financial stress.
- Help you navigate the healthcare system and social services you may be connected tor seeking to connect with.
- Support you in completing application forms for programs or services you are referred to, as needed.
- Build your confidence and skills to better understand available supports and manage your own health and wellness.
Your Role (Patient):
- We encourage you to be informed, engaged, and proactive in your healthcare decisions. Together we can work toward the best outcomes for your health.
Continuity of Care:
Due to the nature of our work and the sensitive information shared, our practice is to keep you with the same social worker throughout your care. This reduces the need for you to repeat your story and helps prevent errors or miscommunication. If you have any concerns about the services you are receiving or would like to request a different social worker, please contact our patient relations department at patientrelations@mosaicpcn.ca.
Completion of Service:
- Follow up appointments are scheduled as needed until your social work goal(s) are met.
- If you are not actively working with social work for a 6 month period, your file will be discharged.
- Your PCP or another Mosaic PCN staff can refer you back if you have new social work goals in the future.
Urgent Services / Resources:
- For urgent or walk-in social service support, call 211.
- If you are in crisis and need to speak to someone, call the Distress Centre at 403-266-4357.
- For emergencies, call 911.
