Get help managing your diabetes
Mosaic PCN offers free access to our Diabetes Optimization Team. The team is made up of pharmacists, nurses, and dietitians who hold specialty Diabetes Educators Certification, with advanced skills and training.
All have extensive experience supporting complex cases and patients to reach health goals.
Our Diabetes Optimization Team can help if you feel you are needing more support with your Diabetes management including:
- Insulin adjustments
- Medication management
- Diet and lifestyle management
- Carb counting
- HbA1C not in target or not improving
How to access diabetes optimization team services through Mosaic PCN
You can be referred to our program by your family doctor. Once your referral is received, you can expect a call from a Mosaic PCN staff member to book your appointment.
