Practicum Student Placement Program

Mosaic Primary Care Network’s (PCN) Practicum Student Placement Program offers students the opportunity to gain hands-on experience in a primary care setting. Students work alongside experienced Mosaic PCN advisors, supporting patients through services that improve quality of life and strengthen community connections. 

Placement Experience 

By participating in a practicum student placement, you will: 

  • Apply academic learning in a real-world healthcare setting.
  • Work under the guidance of an experienced advisor.
  • Support patient assessments, referrals, and care planning. 
  • Participate in team-based care and case discussions.
  • Develop practical skills in communication, documentation, and patient support. 

Practicum Offerings 

At Mosaic PCN, we offer practicum placements in two of our programs: 

  • Social Work
  • Mental Health

Eligibility Requirements

  • Are from an approved educational institution  that has entered into an agreement with Mosaic PCN to complete required practicum training. (Please reach out to your institution’s practicum coordinator if you are unsure).
  • Meet practicum placement requirements of your post-secondary institution. 
  • Must be legally entitled to work and study in Canada.
  • Must fulfill requirements outlined by Mosaic PCN.
  • Social work practicum students must have completed a first BSW practicum. 

Screening Requirements 

Selected students in Mosaic PCN’s Practicum Student Placement Program may be required to provide the following: 

  • Students must provide a Current Police Information Check (PIC) including a Vulnerable Sector Search, at their own expense. The results of the PIC may impact their eligibility for placement. 
  • Immunization documentation is not collected by Mosaic PCN as a standard requirement. If required, this will be determined based on the educational institutions’ practicum policies. 

How to Apply – Social Work 

Students interested in applying for a social work practicum placement should follow the below steps: 

  • Contact your post-secondary institution’s practicum coordinator.
  • Ensure your resume and brief cover letter is current and ready for submission.
  • Your post-secondary institution will submit the practicum request to Mosaic PCN’s Supervisors for the respective program accordingly.

Note that interested students should not email or contact Mosaic PCN directly for application submissions.

Students interested in applying for one of our practicum placements should follow the below steps: 

  • Contact your post-secondary institution’s practicum coordinator.
  • Ensure your resume and brief cover letter is current and ready for submission.
  • Submit your resume to practicum@mosaicpcn.ca.
