Strengthening Patient Care Through Community Connections
An important part of Mosaic Primary Care Network’s (MPCN) role is connecting patients with services in the community. We work closely with various organizations that offer support programs, including recreation programs for children and families, employment and financial coaching, housing information, and help with food and essential services.
On September 9, 2026, Mosaic PCN nurses and pharmacists visited the Centre for Newcomers and The Salvation Army Community Resource Centre with the common goal of expanding patient care.
Fobete Dingha, Community Relations & Engagement Specialist at the Calgary Centre for Newcomers, says visits like these help both organizations work together to better support patients and the community.
“The visit to our centre highlighted opportunities to strengthen referrals, improve access to health care services, and enhance collaboration,” Fobete says. “When community organizations like ours and healthcare providers work closely together, everyone benefits.”
Karen Bautista, Nurse Supervisor at Mosaic PCN, says that the experience of being on-site at community resource centres makes a big difference to patient care.
“Knowledge is power,” Karen says. “Experiencing what community resources offer is more impactful than reading about it in a brochure. Being onsite and developing relationships with resource partners makes our work more thorough and effective.”
Mosaic PCN is committed to creating healthier, stronger, and more connected communities. The organizations listed below offer a range of community-based programs and services.
