Mosaic Primary Care Network (PCN) is a group of family doctors, nurse practitioners, and healthcare professionals that provide primary health care, in partnership with Alberta Health Services. Mosaic PCN member clinics are located in northeast and southeast Calgary.

Mosaic PCN’s Integrated Care Optimal Navigation (ICON) program connects patients without a primary care provider to essential healthcare services.

What can ICON do for you?

ICON can help improve your access to primary care by offering guidance, support, and a pathway to a medical home. Research shows that having a primary care provider leads to better health outcomes.

Managing your healthcare needs
Accessing essential healthcare services

How to Access ICON Services:

Patients can self-refer to ICON by visiting our clinic, or by calling 403-250-5066 to request assistance finding a primary care provider. Our team is ready to connect you with the healthcare services you need.
