Mosaic PCN offers free access to a nurse specializing in diabetic foot care treatment.
For diabetic and pre-diabetic patients, our foot care services include prevention of care complications, educating, and supporting self-care management. Registered nurses and licensed practitioners can also help facilitate access to community foot care programs.
We offer one-on-one appointments and provide individualized care plans with follow-ups scheduled as needed.
How to access our foot care services through Mosaic PCN
Patients must be referred into our foot care services through their family doctor and must fit the eligibility requirements that include patients with diabetes or pre-diabetes who are unable to look after own foot care and have non-complex foot issues. Patients will be required to attend a mandatory diabetic foot care class prior to receiving foot care services.
