Mosaic PCN’s Sprout program “Supporting Postpartum Mothers and Newborns Without Attachment” is a nurse practitioner led program offering postpartum support for mothers and newborns without a primary care provider.
What can Sprout do for you?
The Sprout program offers care for both you and your newborn within 5-7 days after birth, until six (6) weeks postpartum. The Sprout program serves as short-term medical care and support between leaving the hospital and finding you a permanent primary care provider.
Our team will support you with:
- Follow-up care after birth: Regular check-ups for you and your newborn during the first six (6) weeks postpartum.
- Health screening: Important health checks to make sure you and your baby are doing well.
- Learning and guidance: Teaching you everything you need to know about caring for your newborn and supporting your own recovery.
- Connecting you: You’ll be connected with our ICON team, who will help ensure you and your newborn are matched with a trusted primary care provider.
