Welcome to Mosaic PCN: Mental Health Services

Welcome to Mosaic Primary Care Network (PCN) mental health services. You have been referred to our mental health services by your Primary Care Provider (PCP) or another Mosaic team member. Our team works together to provide primary care support as part of your medical home. This document will help you learn what Mosaic PCN mental health therapists do and how they can work with you to meet your health goals.

Appointments and What to Expect:
  • Short-term, goal-focused therapy: Up to six (6) sessions, each lasting about 45 minutes.
  • Counselling may bring up uncomfortable feelings, thoughts, or memories at first. Over time, most people find that the positive changes and benefits outweigh this temporary discomfort.
Services NOT Provided:
  • Urgent or crisis support
  • Assessments and diagnoses
  • Third-party reporting (i.e. for WCB, insurance and/or legal claims).
Cancellation and No-Show Policy:

If you cannot make a scheduled appointment, please call 403-276-1555 (extension 1) at least 24 hours in advance to cancel or reschedule. This allows us to offer that time to another patient who needs support. If you miss an appointment without cancelling or rescheduling, we will consider this a “no show”. After two (2) no-shows, you will be discharged from Mental Health services and require a new referral from your PCP.

Limits to Confidentiality and Sharing Information:

Your health privacy is of the highest importance to us. We use an electronic medical record system (EMR) to chart your health appointments in accordance with the Health Information Act of Alberta. Information collected during your appointment may include the concerns you present, your health goals, provider recommendations and the outcomes. Your PCP receives a copy of all notes, assessment, or report that we write in your EMR. Beyond sharing your session notes with your PCP, what you discuss with us remains confidential.

Note: If you have any concerns about your PCP receiving session notes or there is change to your PCP, please speak to your therapist.

Legally your therapist may be required to give information about you to an appropriate authority. This includes situations where:

  • You are at risk of harming yourself or someone else.
  • You report the abuse or neglect of a child/minor or vulnerable person (including adults with disability).
  • Mosaic PCN’s records are subpoenaed by the courts.
  • Questions regarding your health records can be directed to privacy@mosaicpcn.ca.
Role of your Mosaic PCN Mental Health Therapist:
  • Assess your situation to uncover your strengths, needs, and the goal(s) you want to achieve. Choosing the therapy approach that fits you best, including teaching helpful skills to manage emotions, behaviors, and thoughts.
  • Offer a non-judgemental safe space for open expression and self exploration.
  • Help you navigate appropriate community resources as needed.
  • Clinicians follow the Health Professions Act and the rules set by their professional regulatory college.
Your Role (Patient):
  • We encourage you to be informed, engaged, and proactive in your healthcare decisions. Together we can work toward the best outcomes for your health.
Continuity of Care:

Due to the nature of our work and the sensitive information shared, our practice is to keep you with the same Mental Health Therapist throughout your care. If you have any concerns about the services, you are receiving please contact our patient relations department at patientrelations@mosaicpcn.ca.

Completion of Service:
  • Follow up appointments are scheduled until the goal is achieved to a maximum of six (6) sessions.
  • We encourage you to continue working with your Mental Health Therapist. If there is no activity for six (6) months, your file will be discharged.
Urgent Services / Resources:
  • For urgent or walk-in social service support, call 211.
  • If you are in crisis and need to speak to someone, call the Distress Centre at 403-266-4357.
  • For emergencies, call 911.
  • Rapid access counselling: www.communityconnectyyc.ca.
