Community Health Navigation Services are offered as part of Mosaic Primary Care Network to help patients with long-term health conditions navigate their care and access available services in the community. The program aims to empower patients so that they may achieve self-management and quality of life goals, and ultimately improved satisfaction and health outcomes.
CHNs are not health professionals like a doctor or nurse, but they support the work health professionals do. They are experts in the community they serve and teach patients how to navigate health programs and resources.
How can CHNs support patients?
The services are goal oriented, with discharge once a patient has completed a specific goal. Patients may be referred to our CHNS through any of our Mosaic PCN multi-disciplinary team members.
