Mosaic Primary Care Network (PCN) is a group of family doctors and healthcare professionals that provide primary health care, in partnership with Alberta Health Services. Mosaic PCN member clinics are located in northeast and southeast Calgary.

*Mosaic PCN uses Citrix ShareFile for secure data sharing and data upload. Data are encrypted during transmission, and access to your records is restricted to authorized members of Mosaic PCN.

Proof of vaccination records are collected in accordance with Section 11(1) of the Personal Information Protection Act (PIPA) and shall be used, accessed, and disclosed in accordance with privacy legislation. If you have any questions or concerns about this, please contact hr@mosaicpcn.ca.
