Your Mosaic PCN health care professional will help you create a plan of care specific to your needs. Once your care plan is established, our primary health care team will provide ongoing support and follow-up to help you achieve your health goals.

Your Primary Health Care Team can provide education, motivational support, guidance, and specialty programs to assist with issues such as:
- Diabetes
- High blood pressure and cholesterol
- Pain management
- Exercise education and support
- Tobacco use
- Healthy eating and weight management
- Specialty diets (e.g. celiac, nutritional deficiencies, allergies, etc.)
- Medication management
How to access Mosaic’s primary health care team
Once your referral from your family doctor is received, you can expect a call from a Mosaic PCN staff member to book your appointment and let you know what clinic it will be taking place at.
